During puberty, elevated levels of hormones, especially testosterone, stimulate sebaceous glands to produce more oil, also called sebum, which is secreted through the hair shaft to lubricate and protect the skin. Sebum combines with dead cells that are shed from the walls of these glands, forming a plug at the skin pore. Whiteheads or blackheads are clogged pores.

Pimples form when bacteria grow under clogged pores in the mixture of sebum and cells. The trapped mixture leaks into adjacent tissues, causing inflammation, redness, and pus. The amount of inflammation depends on your immune system.

Acne usually occurs on the face, neck, shoulders, back, or chest. Pimples that are large and deep are called cystic lesions. Cystic lesions can cause painful infections and lead to horrible scarring.

The bacteria usually found in pimples are normally found on the skin, although people with extra sebum usually have more of these types of bacteria on their skin than people with less oil. Sometimes medications can cause acne to develop. This type of acne usually clears up when the medication is stopped.
